Services Provided

Medical:  Most acute illnesses can be handled in the Clinic.  Many chronic illnesses can be followed and monitored in our facility.  Caldwell Health Access Program (CHAP) is also available for chronic diseases.

Caldwell Health Access Program (CHAP):  This award-winning program pairs one patient with one volunteer physician to improve continuity of care and health outcomes.  Patients become part of the physician's private practice for a $5 co-pay per visit and still have access to ancillary services and prescription medications through Helping Hands Clinic.  CHAP does involve case management and requires its patients to be motivated and compliant.

Pharmacy:  Medications are available to treat most illnesses, but specific medications may be limited.  If a physician agrees, a similar drug may be substituted.  TRANQUILIZERS AND NARCOTICS ARE NOT AVAILABLE FROM THE PHARMACY.

Lab and X-Ray:  Most tests ordered by doctors are done without charge by Caldwell Memorial Hospital.

Referrals:  If a referral is needed, efforts are made to find someone who will see the patient at no charge.  If this cannot be done, the patient is responsible for the bill.  HOSPITALIZATION IS NOT COVERED.

Education:  Patient education classes are available for diabetes, general nutrition and tobacco education.  If you have been referred to one of these classes, you are required to complete them in order to receive clinic services.

Bob Ashley Mental Health and Substance Abuse Program:  Intensive case management, group therapy and individual counseling are available with referral from a primary care physician.  The program coordinator will work with each patient's primary care physician to insure medication compliance and improve integration of care.

Surgery:  Limited surgical procedures are available upon approval.

Patients are asked to contribute $5 per visit to help the clinic cover its basic operating costs and to take ownership in their care.

Immunizations

Pre-natal care, Pap Smears, or Family Planning

Dental Work

These services are available through the Caldwell County Health Department or private providers.
