Caldwell Health Access Program (CHAP) is also available for chronic diseases. Caldwell Health Access Program (CHAP): This award-winning program pairs one patient with one volunteer physician to improve continuity of care and health outcomes. Patients become part of the physician's private practice for a $5 co-pay per visit and still have access to ancillary services and prescription medications through Helping Hands Clinic. CHAP does involve case management and requires its patients to be motivated and compliant. Pharmacy: Medications are available to treat most illnesses, but specific medications may be limited. If a physician agrees, a similar drug may be substituted. TRANQUILIZERS AND NARCOTICS ARE NOT AVAILABLE FROM THE PHARMACY. Lab and X-Ray: Most tests ordered by doctors are done without charge by Caldwell Memorial Hospital. Referrals: If a referral is needed, efforts are made to find someone who will see the patient at no charge. If this cannot be done, the patient is responsible for the bill. HOSPITALIZATION IS NOT COVERED. Education: Patient education classes are available for diabetes, general nutrition and tobacco education. If you have been referred to one of these classes, you are required to complete them in order to receive clinic services.
